Last names in Dungeons & Dragons follow race first and region second. The Player's Handbook gives each ancestry its own surname logic: human names in the Forgotten Realms mimic English compound surnames (Amblecrown, Tallstag, Greycastle), while Calishite, Mulan, Rashemi and Turami humans borrow Arabic, Egyptian, Slavic and Italianate shapes like Pashar, Anskuld, Dyernina and Marivaldi. Elves use flowing family names such as Galanodel and Siannodel; dwarves carry blunt two-part clan names like Battlehammer and Fireforge; halflings sound like cottages and gardens (Tealeaf, Goodbarrel, Underbough); gnomes have clan names like Nackle and Timbers; dragonborn use long ancestral clan names such as Kepeshkmolik; goliaths take hyphenated clan names like Katho-Olavi.

This list mixes canon examples from the core books with originals built on the same patterns, so you can grab a surname that sounds like the Sword Coast without repeating the handful every table already uses. There are also occupational names for commoners (Fletcher, Wainwright, Chandler), noble houses for Cormyr-style courts, and coastal, northern and Germanic-flavored surnames for regional color.
