It depends on the setting. In Warcraft and most D&D worlds, orcs carry an earned epithet or a clan name rather than an inherited surname (Hellscream, Saurfang, Bloodfist). In The Elder Scrolls they use patronymics with gro- for sons and gra- for daughters. Tolkien's orcs are usually known by a single name only, so a last name there is a later fantasy convention.

Two blunt, concrete words that describe a deed, a wound, a weapon or an animal: Ironhide, Elfbane, Brokentusk, Wolfrider. Keep it to two or three syllables, favour hard consonants, and make it something the orc could plausibly have earned. Names that only list adjectives (Darkshadow, Grimdoom) sound weaker than ones built from nouns.

Half-orcs raised by humans usually take the human parent's surname or a simple orcish family name. Those raised by the tribe earn an epithet like any other orc. A common compromise is a guttural one-word name (Karguk, Urzul, Ghorzak) that sounds orcish without boasting.
