A Great Old One is a very specific kind of god. In Lovecraft's Mythos the term covers the immensely old, immensely powerful beings bound to a place on or under the Earth: Cthulhu dreaming in sunken R'lyeh, Tsathoggua squatting in the caverns of N'kai, Ithaqua walking the northern ice, Yig coiled beneath the plains. Unlike the abstract Outer Gods, they have bodies, addresses and cults.

That gives the true name a clear brief: heavy consonant clusters (gg, th, kk, zh), doubled vowels, and a hyphen or apostrophe that splits it into chanted halves, the way Yog-Sothoth and Rhan-Tegoth do.
