Choosing a name is one of the first concrete steps in many transitions, and it works differently from naming a baby: you're naming an adult you already know. Most trans people land on a name in one of a few ways. Some keep a link to their birth name—the same initial (Michael to Michelle, Rebecca to Beck), a gender-swapped version (Julian to Julia, Victoria to Victor), or a name that starts with the same sound—which makes the change easier for family and keeps monograms and signatures intact. Others choose a name their parents considered, a grandparent's name, or a name from a book or song that stuck. And plenty simply try names on for a few weeks in a game, a Discord server or with close friends until one feels like their own.

The names trans people actually pick cluster in recognisable ways. Trans women often gravitate toward classic, soft names that were popular for girls in their own birth year—Lily, Emily, Alice, Charlotte, Sophie, Zoe—so the name fits their age. Trans men lean toward warm, slightly vintage names like Elliot, Theo, Ezra, Milo, Jasper and Felix. Nonbinary people and those still exploring often choose neutral names (Rowan, Ash, Sage, Quinn, Riley, Avery) or nature words (Wren, River, Juniper), which also serve well as an interim name during questioning.
