Russian bratva figures earn short, blunt handles (The Wolf, The Butcher, Grandfather), Yakuza bosses are addressed as oyabun with formal surname-first names, and Irish mob chiefs pick up parish nicknames like The Bishop or The Deacon. Modern kingpins, real and fictional, tend toward a plain name made fearsome by reputation: Frank Lucas, Marlo Stanfield, Gus Fring.

For a fictional boss, pair a surname that fits the setting with one nickname that tells the reader how he got the chair, then let everyone address him differently: subordinates say "Mr. Ferrante," rivals say "the Butcher," his mother says "Ottavio."
